Most riders think a sore hip needs a softer saddle. It is a logical thought, but it points in the wrong direction. Arthritis limits how far the hip can flex before bone meets bone. Padding cannot increase that range. The shape of the saddle can.
I have watched this play out in bike fits more times than I can count. A rider shows up with a thick gel saddle, convinced that more cushioning will fix the pain. The first time they pedal, the pelvis still sits in the same position. The hip still hits the same end range. And the thicker pad has already started to push up into soft tissue. We take the gel saddle off the bike, and the rider's expression says it all: the pain is still there, just wrapped in foam.
Arthritis Changes the Mechanical Contract
Osteoarthritis does two things that matter for saddle fit. It narrows the joint space inside the hip, and it reduces the amount of pelvic tilt a rider can hold without loading bone against bone. Some men lose internal rotation first. Others lose flexion. Most lose a mix of both, and usually more on one side than the other.
A fixed saddle cannot account for asymmetry. It offers one width and one profile. The left hip and the right hip are not the same after years of wear. If the saddle forces both sides to sit identically, the tighter hip either fails to clear the saddle's front or the rider shifts in the saddle to buy back range. That shift loads the lumbar spine and the knees. The pain moves.
Why More Padding Makes Things Worse
Padding manages load over time. It does not manage joint range. A soft saddle absorbs some pressure but also compresses. Under body weight, the sit bones sink into the pad, and then the middle of the saddle pushes up. That upward pressure is exactly what an arthritic hip does not need: more contact against the perineum while the pelvis tries to rotate forward.
There is another problem. A thick pad adds lateral instability. The pelvis rocks slightly on every pedal stroke, and the small stabilizer muscles in the hip fire constantly to hold position. Over two hours, those repeated corrections load a joint capsule that is already inflamed. The rider finishes the ride with deep hip pain and blames the saddle. The saddle was never the stiffness problem. The shape was.
What Adjustable Shape Changes
Bisaddle builds saddles as two independent halves mounted on sliding rails. That single design choice changes the conversation from "which fixed shape hurts least" to "which position eliminates the pinch." You move each half side to side. The rear width adjusts from about 100 mm to 175 mm. You can also tilt each half independently, which means the left support surface does not have to match the right.
That independent tilt matters for arthritic hips. A rider with right hip osteoarthritis often sits with the right foot externally rotated and the right knee tracking outward. The central split also creates a channel that runs down the middle of the saddle. The width of that channel changes as the halves move apart or together. You can make it narrow for a road position or wide for a more upright, arthritis-friendly posture.

A Setup Sequence for Arthritic Hips
The setup process is simple but requires patience. Arthritis responds to sustained load, so a setting that feels fine at minute five may reveal itself at minute forty. Here is the process I use with riders:
- Measure sit bone width with a cardboard strip. Sit on it, find the two deepest impressions, and measure center-to-center.
- Set the Bisaddle halves slightly wider than that measurement. The extra width gives the pelvis a stable shelf when hip flexion is limited.
- Tilt each half independently. If one hip has less flexion, lower the rear of that side enough to clear the joint's end range.
- Ride for 20 minutes on flat ground. Note any pinching at the front of either hip.
- Adjust in small increments. Then test on a climb. Climbing demands more hip flexion and will expose any remaining restriction.
- Lock the position only after pain-free flat and climbing rides.
Adjustability Is Ongoing Arthritis Care
Arthritis changes. A saddle that fits one season can hurt the next if hip flexion worsens or the rider changes crank length, stem height, or handlebar reach. A fixed saddle means buying another saddle and hoping. Bisaddle means loosening the adjustment bolts, moving the halves, and retesting. The saddle follows the rider's body as the body changes.
The saddle is the one contact point that supports the entire trunk. For a man with osteoarthritis, the goal is not to find a single perfect shape. The goal is to keep the saddle matched to the body as the body changes. Width, tilt, central channel, front pressure: all of it can be re-tuned. Arthritis narrows the margin for error. Padding cannot widen that margin. Adjustable shape can.
